Can Boeing Trust Pilots?
"What’s critical to the current, mostly uninformed discussion is that the 737 MAX system is not triply redundant. In other words, it can be expected to fail more frequently than one in a billion flights, which is the certification standard for flight critical systems and structures.
What Boeing is doing is using the age-old concept of using the human pilots as a critical element of the system. Before fly-by-wire (FBW) came along, nearly all critical systems in all sizes of airplanes counted on the pilot to be a crucial part of the system operation.

Though the pitch system in the MAX is somewhat new, the pilot actions after a failure are exactly the same as would be for a runaway trim in any 737 built since the 1960s. As pilots we really don’t need to know why the trim is running away, but we must know, and practice, how to disable it.

What makes that such a tall order is that FBW airplanes – which include all the recent Airbus fleet, and the 777 and 787 from Boeing – don’t rely on the pilots to handle flight control system failures. FBW uses at least a triple redundant computer control system to interpret the inputs of the cockpit controls by pilots into movement of the airplane flight controls, including the trim. If part of the FBW system fails, the computer identifies the faulty elements and flies on without the human pilots needing to know how to disable the failed system.
I know, I know, you’re yelling, “What about the Air France Airbus over the Atlantic?” It was determined that all three pitot tubes on the airplane froze robbing the FBW system, and the human pilots, of airspeed and other air data at night and in an area of thunderstorms. The pitot tubes were thought to have enough heat to prevent that, but those extreme conditions were outside the certification envelope. And certification theory also thought the engines on the Airbus Sully and Skiles were flying were far enough apart that large birds wouldn’t take out both. Was that one in a billion? Probably.

After three of these automatic trim occurrences, the First Officer stated that the control column was too heavy to hold back and the Captain declared a PAN to ATC advising instrument failure and requesting to remain on runway heading. This was approved. The controller asked if they wanted to return and received a ‘Standby’ response. The Captain then moved the STAB TRIM switches to CUT OUT, which solved the problem, and he decided to continue and complete the flight without engaging the AP and using manual trim. Three Abnormal Checklists were completed - ‘Airspeed Unreliable’, ‘ALT DISAGREE’, and ‘Runaway Stabiliser’ "

"The company said it will change the MCAS software to give the system input from more than one AOA sensor. It will limit how much MCAS can move the horizontal tail in response to an erroneous AOA signal. And when activated, the system will kick in only for one cycle, rather than multiple times.
Unless some other cause for the crashes emerges, 737 MAXs will remain grounded until that software patch is fully deployed and approved to have fixed the problem."

The safety review also forgets to mention that the system can reset itself after each pilot input, ignoring the fact that it is able to repeatedly push the plane’s nose down.
Finally, the MCAS seem relies on the reading of a single sensor while its potential failure was evaluated as one level below “catastrophic”. This goes against the usual FAA regulations. Data from the FDR showed that “the two sensors differed by some 20 degrees not only throughout the flight but also while the airplane taxied on the ground before takeoff”.There wasn’t a complete and proper review of the documents,” a former FAA engineer is quoted saying. “Review was rushed to reach certain certification dates”.

Na kol Boeing žaidė su 737s, Airbus paleido a320 Neo. Boeing labai sunerimo, nes visi perka a320, tad laiko kurti naują lėktuvą, kaip ir nėra.
Ant seno korpuso užmetė naujus variklius, pastebėjo, kad kylant lėktuvas nėra visai stabilus - pačiu svarbiausių momentu, kai stabilumas yra reikalingas.
Instaliavo MCAS, kad suskleidus flaps us, kuomet sumažėja keliamoji galia įsijungtų MCAS. Na jei netyčia tie per dideli varikliai pradėtų kelti nosį, tai MCAS veiktų kaip saugiklis.
Laikas spaudžia, atiduoda FAA sertifikavimui, FAA atiduoda testavimo darbus kam? Ogi Boeing ir turbūt dar sumoka. Na tebūnie America great again. O kaip dėl pastebėtų testavimo klaidų ? Nieko rimto? - gal kokį update išleisime, kada.
Užsakymai pilasi, nukrenta Lion air, Boeing kaltina pilotus, juk šiaip ar taip Indonezijoje kas antrą dieną tie lėktuvai krenta, o čia naujas MAX. Boeing pasako, kad sukurs update, užsakymai pilasi toliau.
O čia dar antras nusivožia, su identiška charakteristika, nežinau ar update padės.

Čia rašo apie kažkokią papildomai įsigyjamą įrangą, įspėjančią apie klaidingus kilimo kampo rodmenis. But according to reports, the Lion Air plane did not have an optional feature installed in the aircraft—one that Boeing doesn’t provide as a standard feature and costs additional money to install—which would have alerted crew that the AOA readings were erroneous and likely to trigger the MCAS system unnecessarily. That’s because Boeing and the FAA didn’t require what was called the “AOA DISAGREE” alert be installed for safe operation of the 737 Max.
Reuters reported in November that several airlines, including American Airlines, Southwest Airlines, Canada’s WestJet, SilkAir and flydubai, had the alert installed before the Lion Air crash. Teisingai, dėl paties AF447 gedimo A330 neneria žemyn nosim. Klaida buvo antrojo piloto, kuris traukė vairalaizdę į save.

AF447 nelabai čia į temą, gedimas su kuriuo iki tol ir po to susitvarkė daugybė įgūlų. Ten tiesiog buvo visiškai nekompetetingas kopilotas Bonin'as ir kartu sukrito labai daug nedėkingų aplinkybių (oro sąlygos, kapitono poilsio metas, Airbus valdymo dizainas, nesusikalbėjimas kritiniu momentu...). Apskritai tragiškas nutikimas, nes įvyko nekylant ar leidžiantis ir buvo galybė laiko jo išvengti.

Kodėl turėtų meluoti? Ethiopian yra vienos geriausių avialinijų pasaulyje su nauju lėktuvų parku ir dideliu dėmesiu saugai. Ar vien dėl to, kad jie iš Afrikos iš karto yra metama dvejonė?

Ethiopian, žinoma, su tokiomis išvadomis nesutiko:

Nuo teroro kentėjo ne kartą, tikrai turi pagrindą abejoti.
Dabar dirba didelė tyrėjų komanda, viso pasaulio dėmesys atkreiptas - pritempti ar kažką nuslėpti bus sunku.

Nesigilinant į rasinius/kultūrinius skirtumus, Lion Air ir Ethiopian Airlines turi šį tą bendra: labai spartų augimą pastaraisiais metais. O spartus augimas reiškia pilotų/mechanikų trūkumą ir galimai prastesnį jų paruošimą.

Pilots now spend more time learning automated systems than practicing hands-on flying, so newer pilots are less comfortable with taking manual control when the computer steers them wrong, according to interviews with a dozen pilots and pilot instructors at major airlines and aviation universities around the world. "The automation in the aircraft, whether it's a Boeing or an Airbus, has lulled us into a sense of security and safety," said Kevin Hiatt, a former Delta Air Lines pilot who later ran flight safety for JetBlue. Pilots now rely on autopilot so often, "they become a systems operator rather than a stick-and-rudder pilot."
As a result, he said, "they may not exactly know or recognize quickly enough what is happening to the aircraft, and by the time they figure it out, it may be too late." [...] While automation has contributed to the airline industry's stellar safety record in recent years, it has also been a factor in many of the crashes that have still occurred around the world. A 2011 study by a federal task force found that in about 60 percent of 46 recent accidents, pilots had trouble manually flying the plane or handling the automated controls. Complicated automation systems can also confuse pilots and potentially cause them to take action they shouldn't, pilots said.

Bet su drausme nekas ... Kvaišalus galima pamatuoti, na o dėl šio - skaistybės triusikus privaloma tvarka?
Bet kokiu atveju - skrydžio metu turėjo privalomai pasiilsėti.

The Ethiopian Airlines CEO said Tuesday that both pilots of Flight 302 had been trained and briefed on the FAA directive after the Lion Air crash.
"There was training" with emphasis "on MCAS and also on the flight controls in general," he said.
Kodėl turėtų meluoti? Ethiopian yra vienos geriausių avialinijų pasaulyje su nauju lėktuvų parku ir dideliu dėmesiu saugai. Ar vien dėl to, kad jie iš Afrikos iš karto yra metama dvejonė?

Dėl Toyotos dar nebaigtas tyrimas, gal bus nustatyta, kad algoritmas teisingas ir pagalvės nebūtų padėjusios, o čia jau kaip ir faktas ir pats Boeing pripažino, kad kažkas su orlaiviu negerai, tik neaišku, ar užteks tik softą atnaujinti ar ir dizainą keisti.Paskutinis taisė Lettered; 2019.03.15, 10:07.
